I have been running quick races at Monza, with me starting at the rear of
the field. I find I am getting a stop and go penalty for cutting the track
through the first turns when I am not cutting the track. There will be a
pile up in front of me, I slow down and weave through the cars, yet staying
on the track as I do this. About 50% of the time I will get a stop and go
penalty for cutting the track.
Is this happening to others?

As soon as I set one wheel on grass, I get the black flag....

The only time I seem to get a black flag for off course is when I gain
position. The game tracks your passes (all those "great move" messages) so
presumablely a simple routine that A)checks your position, B) checks to see
if other passed car is on the racing surface and either issues "great pass"
or blag flag would do it.
